To register your child for dance classes; please download the below registration form, then print and mail the registration form along with the requested payments to the address indicated at the bottom of the form.  You also have the option to fax your registration form to (904) 880-5328 and pay your registration fee plus first month's tuition online.  When paying online, indicate your dancers name and preschool.    Remember, your dancer must be 3 to 6 years old to participate in our dance program.   DO NOT send your registration form to your preschool.   Registration is closed from March 1st until June 1st (due to recital conflicts).  We re-open registration during the summer prior to our season starting in September.    After you have finished with the registration form, use your BACK button to return to our website. 2010-2011 Registration Form Thank you for allowing your child to dance with us!
